ABSTRACT

Introdução: a colelitíase é uma das doenças mais comuns do trato digestivo, acometendo 6-10% da população adulta. Destes, aproximadamente 8- 20% apresentam coledocolitíase associada. A estratificação de risco de coledocolitíase pré-operatória nos portadores de colelitíase busca subsidiar a definição de uma terapêutica mais adequada, em tempo hábil, para cada caso. Objetivo:estratificar o risco pré-operatório de coledocolitíase em pacientes submetidos a colecistectomia em um hospital terciário. Metodologia:estudo observacional descritivo, retrospectivo, a partir da análise de prontuários de pacientes submetidos a colecistectomia por cálculos biliares em um hospital público em João Pessoa, Paraíba, entre agosto de 2021 e janeiro de 2022, durante a pandemia de COVID-19. A estratificação de risco foi realizada a partir do estabelecido pela Sociedade Americana de Endoscopia Digestiva (ASGE) em 2010. Resultados: foram selecionados 41 pacientes, a maior parte do sexo feminino, com média de idade de 49,6 anos, em sua maioria submetidos a colecistectomia videolaparoscópica, sem colangiografia intraoperatória. O exame de imagem mais realizado no pré-operatório foi a ultrassonografia de abdome. De acordo com os critérios da ASGE, 18 (43,9%) pacientes foram classificados como baixo risco, 19 (46,4%) foram estratificados no risco intermediário, e 4 foram de alto risco (9,7%). Conclusão: apesar de uma amostra de tamanho limitado, evidencia-se frequência relevante do risco alto ou intermediário de coledocolitíase em portadores de colelitíase. A estratificação de risco para coledocolitíase é uma ferramenta importante a ser utilizada de rotina no pré-operatório da colecistectomia nos pacientes com diagnóstico de colelitíase.


Introduction: gallstones are one of the most common diseases of the digestive tract, affecting 6-10% of the adult population. Of these, approximately 8-20% have associated choledocholithiasis. Preoperative risk stratification of choledocholithiasis in patients with cholelithiasis seeks to allow setting of a proper therapy, in a timely manner, for each case. Objective: stratify preoperative risk of choledocholithiasis in patients undergoing cholecystectomy in a tertiary hospital. Methods: descriptive, retrospective observational study, based on the analysis of medical records of patients undergoing cholecystectomy for gallstones in a public hospital in João Pessoa, Paraíba, between August 2021 and January 2022, during COVID-19 pandemic. Risk stratification was performed based on American Society for Digestive Endoscopy (ASGE) criteria from 2010. Results: 41 patients were evaluated. Most of them were female, with a mean age of 49.6 years, most of whom underwent laparoscopic cholecystectomy without intraoperative cholangiography. The most common imaging performed preoperatively was abdominal ultrasound. According to ASGE criteria, 18 (43.9%) patients were classified as low risk, 19 (46.4%) were stratified into intermediate risk, and 4 scored high risk (9.7%). Conclusion: despite a limited sample size, there is a relevant prevalence of high or intermediate preoperative risk of choledocholithiasis in patients with cholelithiasis. Risk stratification for choledocholithiasis is an important tool to be routinely used in the preoperative period of cholecystectomy in patients with diagnosis of cholelithiasis.

ABSTRACT Objective: to determine the prevalence of incidental gallbladder cancer (IGBC) in cholecystectomies performed in a tertiary public hospital and to describe technical and epidemiological aspects of performing cholecystectomies for presumably benign disease. Method: descriptive, retrospective observational study, based on analysis of medical records of patients undergoing cholecystectomy with preoperative hypothesis of benign disease between January 2018 and January 2022. Results: prevalence of gallbladder adenocarcinoma in our sample was 0.16%, similar to data in the literature. Technical aspects during cholecystectomy were also described with a frequency similar to that found in the literature. Conclusion: despite a rare disease, IGBC is relevant in the routine of the General Surgeon. Its diagnosis, staging and treatment directly affect the prognosis. Technical aspects during cholecystectomy are not always remembered by surgeons and can interfere with the prognosis and subsequent treatment of the patient.


RESUMO Objetivos: determinar a prevalência de câncer incidental de vesícula biliar (CIVB) em colecistectomias realizadas em um hospital público terciário e descrever aspectos técnicos e epidemiológicos na realização de colecistectomias por doença presumidamente benigna. Método: estudo observacional descritivo, retrospectivo, a partir da análise de prontuários de pacientes submetidos à colecistectomia com hipótese pré-operatória de doença benigna entre janeiro de 2018 e janeiro de 2022. Resultados: a prevalência de adenocarcinoma de vesícula biliar na amostra avaliada foi de 0,16%, semelhante aos dados da literatura mundial. Os aspectos técnicos durante a realização da colecistectomia também foram descritos com frequência semelhante ao encontrado na literatura. Conclusão: o CIVB é uma doença rara mas de importância relevante na rotina do Cirurgião Geral. Seu diagnóstico, estadiamento e tratamento implicam diretamente no prognóstico. Aspectos técnicos durante a realização da colecistectomia nem sempre são lembrados pelos cirurgiões e podem interferir no prognóstico e tratamento subsequente do paciente.

ABSTRACT

Changes in course of the internal carotid artery (ICA) are uncommon, and dehiscence of the carotid canal with cochlea may occur. A 48-year-old female individual with pulsatile tinnitus. No other otologic symptoms observed. Otolaryngologic examination and audiometric test with normal results. Computed tomography (CT) scan of the mastoid bones showed dehiscence of cochlea with ICA on the right side. An option for monitored observation was made after analysis of the risks and undefined results of surgery. Patient maintained clinical and audiometric profile. Carotid-artery cochlear dehiscence is a condition that must be known, remembered and investigated, because it may mimic other otologic pathologies. Knowledge about it prevents serious complications that can be difficult to reverse.

ABSTRACT Introduction: It is estimated that there are nearly 40 million people with the human immunodeficiency virus (HIV) worldwide. Due to the advent of antiretroviral drugs, it has been observed increasing in obesity and metabolic rates among patients undergoing treatment. Thus, numerous surgical therapies for weight loss are proposed for continuous improvement in health of patients with HIV, being the vertical gastrectomy an option for intact intestinal transit. Objective: To evaluate the applicability of the vertical gastrectomy in patients with morbid obesity and HIV. Methods: Was conducted a systematic review of the literature, in the electronic databases Scopus, Pubmed, Cinahl, Scielo, Cochrane and Lilacs, from 1998 to 2015. MeSH headings used in data collection were "Gastrectomy" and "Morbid obesity" being combined with the descriptor "HIV". Were found 2148 articles in Scopus, 1234 in PubMed and 784 in Cinahl. The articles were analyzed by the Jadad Quality Scale, being reduced to 40 articles, subsequently reassessed using an elaborated form by the Critical Appraisal Skills Programme (CASP), reaching 12 articles in the end. Conclusion: It was found that vertical gastrectomy constitutes a safe and effective method, with low mortality and low rate of postoperative complications, being recommended as surgical technique in patients with obesity, HIV and comorbidities.


RESUMO Introdução: Estima-se que haja quase 40 milhões de pessoas com o vírus da imunodeficiência adquirida (HIV) no mundo. Com o advento dos antirretrovirais, observou-se aumento da obesidade e de taxas metabólicas nos pacientes em tratamento. Assim, inúmeras terapias cirúrgicas para a perda de peso estão sendo estudadas para a melhoria contínua da saúde dos pacientes com HIV, sendo a gastrectomia vertical uma opção de trânsito íntegro. Objetivo: Avaliar a aplicabilidade da gastrectomia vertical em pacientes com obesidade mórbida e HIV. Métodos: Foi realizada revisão sistemática de literatura, de artigos publicados nas bases eletrônicas de dados Scopus, Pubmed, Cinahl, Scielo, Cochrane e Lilacs, no período de 1998 a 2015. Os descritores do MeSH utilizados na coleta dos dados foram "Gastrectomia" e "Obesidade mórbida", sendo combinados com o descritor "HIV". Resultados: Foram encontrados 2.148 artigos na Scopus, 1.234 no Pubmed e 784 na Cinahl. Os artigos foram analisados pela Escala de Qualidade de Jadad, reduzindo para um total de 40 artigos, os quais foram posteriormente reavaliados, utilizando um formulário elaborado pelo Critical Appraisal Skills Programme (CASP), chegando ao montante de 12 artigos ao final. Conclusão: Verificou-se que a gastrectomia vertical constitui método seguro e eficaz, apresentando baixa morbimortalidade e baixo índice de complicações pós-operatórias, sendo técnica cirúrgica segura para uso em pacientes com obesidade, HIV e comorbidades.

ABSTRACT

PURPOSE: To evaluate and compare clinical and inflammatory responses to the surgical trauma caused by cholecystectomy via several access approaches: single-port umbilical incision (SILS), transvaginal natural orifice transluminal endoscopic surgery (NOTES), laparoscopy, and Laparotomy.METHODS: Twenty-eight female pigs were equally divided into four groups and submitted to cholecystectomy by single-port umbilical incision, transvaginal NOTES, laparoscopy, or Laparotomy. An additional five animals served as controls (sham group). Animals were monitored perioperatively regarding anesthesia and surgical procedure times, as well as for the presence of complications. Postoperatively, they were evaluated regarding time to ambulation and feeding, and the presence of clinical events. Procalcitonin, C-reactive protein (CRP), and AQUI feron-gamma (IFN-γ) measurements were performed before surgery and immediately, two days, and seven days after surgery. Animals were sacrificed and necropsied at seven days after surgery.RESULTS: All procedures were successfully performed as proposed in each group. Only minor complications, such as gallbladder perforation and bleeding from the liver bed, were observed during surgery in all groups. The vaginal NOTES group showed higher anesthesia and surgical procedure times compared to the other groups (p<0.001). No other between-group differences in perioperative or postoperative times, clinical evolution, or serum inflammatory markers were observed. Only adhesions were found on necropsy, with no differences between groups.CONCLUSION: The single-port umbilical and transvaginal NOTES access approaches were feasible and safe compared to laparoscopic and laparotomy for cholecystectomy.

ABSTRACT

Neste trabalho objetivou-se avaliar e selecionar progênies de maracujazeiro- azedo com resistência à antracnose em condições de casa de vegetação. O experimento foi conduzido na estação experimental da biologia da Universidade de Brasília. Foram utilizadas sementes (propagação sexuada) coletadas no campo da fazenda Água Limpa. As progênies usadas nesse experimento foram: seleção MAR, 20#01, 20#05, 20#12, 20#15, 20#19, 20#21, 20#36, 20#40 e 20#2005, PL1, PL2, PL3, PL4, PL5, PL7, GA2, BRS Gigante Amarelo, MSC, BRS Sol do Cerrado, RC3, Roxo Australiano, AR01 e FB100. O delineamento experimental foi em blocos casualizados com parcela subdividida com 24 tratamentos (progênies), 4 repetições e 6 plantas úteis por repetição. A progênie MAR20#19 apresentou as menores incidências na folha, planta e severidade, 89,2%, 87,5% e 3,71, respectivamente. A progênie MAR20#19 comportou-se como moderadamente resistente e as outras progênies como altamente susceptíveis.


In this work, the objective was to evaluate and select progenies of passion fruit with resistance to anthracnose disease under conditions of a greenhouse. The experiment was conducted at the experimental station of biology of the University of Brasília. It was used seeds (sexual propagation) collected from the farm Água Limpa field water. The progenies used in this experiment were the selections MAR, 20#01, 20#05, 20#12, 20#15, 20#19, 20#21, 20#36, 20#40, 20#2005, PL1, PL2, PL3 , PL4, PL5, PL7, GA2, BRS Gigante Amarelo, MSCA, BRS Sol do Cerrado, RC3, Purple Australian, AR01 and FB100. The experimental design was randomized block with split plot with 24 treatments (progenies) 4 replicates and 6 plants per repetition. The progeny mar20#19 had the lowest incidences leaf, plant and severity, 89.2%, 87.5% and 3.71, respectively. The progeny mar20#19 was observed as moderately resistant and the other progenies as highly susceptible.

ABSTRACT

A expansão da Estratégia Saúde da Família (ESF) no país depende da participação de profissionais comprometidos com as propostas estabelecidas por esse modelo de assistência. Este estudo busca compreender a motivação dos profissionais em escolher a ESF como área de atuação, e os fatores responsáveis pela permanência e vinculação destes à ESF. Utilizada abordagem qualitativa, foram entrevistados 14 profissionais, de seis diferentes categorias, atuantes nas Equipes de Saúde da Família na Regional de Saúde de Sobradinho, no Distrito Federal. A satisfação por colaborar com o avanço desse modelo de assistência que promove a saúde integral do paciente é o principal fator estimulante para o profissional. Achado outro de grande valia é a crítica à atual falta de apoio e investimento da Secretaria de Saúde local no avanço da ESF.


The expansion of the Family Health Strategy (FHS) in Brazil depends on the participation of professionals committed to the proposals set out by this model of care. This study seeks to understand the motivation of professionals in choosing the FHS as their , and the factors responsible for their affiliating and remaining with the FHS. Using a qualitative approach, we interviewed 14 professionals, from 6 different categories, working in Family Health Teams at the Sobradinho Regional Health center, in the Federal District. The satisfaction from supporting the advancement of this model of care that promotes the overall health of the patient is the principal inspiring factor for the professional. Another finding of great value is the criticism of the current lack of support and investment from the local Health Department in advancing the FHS.

ABSTRACT

OBJETIVOS: Avaliar a epidemiologia e os resultados cirúrgicos do tratamento das fraturas-luxações de cotovelo, incluindo apenas os casos que associam a luxação com a fratura do processo coronoide e da cabeça do rádio (tríade terrível). MÉTODO: Foram avaliados 19 pacientes, sendo 12 do gênero masculino e sete do gênero feminino. Foi feita uma análise dos prontuários para coleta de dados a respeito do mecanismo do trauma, padrão das fraturas, tempo para a cirurgia e tipo de procedimento realizado. Foi realizada a avaliação clínica para mensuração dos arcos de movimento do cotovelo e aplicação do questionário MEPS. RESULTADOS: O mecanismo de trauma mais comum em nossa casuística foi a queda de altura (12 pacientes). Todos os pacientes foram submetidos a tratamento cirúrgico sendo que o tempo médio entre a data do trauma e a cirurgia foi de 16,1 dias. O seguimento médio foi de 50,3 meses. Foram obtidos arcos de flexo-extensão média de 112° e pronossupinação de 127,9°. A média do questionário MEPS foi de 86 pontos, sendo encontrados resultados excelentes e bons em 15 dos pacientes (79 por cento). O tempo para a cirurgia, arco de flexoextensão final maior que 100° e a contratura em flexão menor que 30° se mostraram estatisticamente significantes para o bom resultado clínico final. Cinco pacientes tiveram complicações, sendo três em relação a nervos periféricos, uma pseudoartrose e uma instabilidade recorrente. CONCLUSÃO: Apesar da gravidade da lesão encontrada na tríade terrível do cotovelo, a maioria dos pacientes avaliados obteve um cotovelo estável com bons resultados clínicos. Os fatores que levam a melhores resultados clínicos são: cirurgia antes de 14 dias de lesão, arco de flexo-extensão maior que 100° e contratura em flexão menor que 30°.


OBJECTIVES: To evaluate the epidemiology of and surgical results from treating elbow fracture-dislocations, including only the cases in which dislocation is associated with fracture of the coronoid process and the radial head (terrible trio). METHODS: Nineteen patients were evaluated: 12 males and 7 females. The medical records were analyzed to gather data about the mechanism of injury, fracture pattern, time elapsed until surgery and type of procedure applied. A clinical assessment was made to measure elbow range of motion, and the MEPS questionnaire was applied. RESULTS: The most common mechanism of injury in our sample was a fall from a height (12 patients). All the patients underwent surgical treatment and the mean time elapsed between the date of the injury and the surgery was 16.1 days. The mean follow-up was 50.3 months. The mean range of flexion-extension obtained was 112° and the mean range of pronation-supination obtained was 127.9°. The mean score from the MEPS questionnaire was 86 points, and excellent and good results were obtained for 15 patients (79 percent). The time elapsed until surgery, final flexion-extension range greater than 100° and flexion contracture of less than 30° were shown to have a statistically significant relationship with a good final clinical result. Five patients had complications, of which three cases related to peripheral nerves, one case to pseudarthrosis and one case to recurrent instability. CONCLUSIONS: Despite the severity of the injuries found in the terrible trio of the elbow, most of the patients evaluated here achieved elbow stability with good clinical results. The factors that led to better clinical results were surgery not more than 14 days after the injury, flexion-extension range greater than 100° and flexion contracture less than 30°.

ABSTRACT

A polipose nasossinusal é um processo inflamatório crônico da mucosa nasal, caracterizado pela presença de pólipos nasais múltiplos e bilaterais. Vários tipos de medicações têm sido usados no seu tratamento. Para estudar o resultado de diferentes formas de tratamento, é preciso alguma forma de estadiamento. OBJETIVOS: Apresentar um novo método endoscópico de estadiamento, baseado na endoscopia nasal e na avaliação tridimensional dos pólipos, comparar sua reprodutibilidade entre outros dois métodos já difundidos. Forma de estudo: Estudo de coorte histórica com corte transversal. Material e método: Três examinadores avaliaram exames de 20 pacientes portadores de polipose nasossinusal em diferentes momentos, antes e 15 e 30 dias após o início de um tratamento com prednisona, na dose de 1 mg/kg/dia por 7 dias. Foi avaliado o grau de concordância entre os examinadores para cada método, utilizando-se o Kappa múltiplo para análise estatística. Resultados: Os três métodos mostraram-se reprodutíveis, sendo que o método proposto apresentou menor concordância entre os examinadores. Conclusão: O estadiamento proposto mostrou-se reprodutível, apesar de apresentar menor concordância do que os outros dois estadiamentos.


Nasal Polyposis is a chronic inflammatory process of the nasal mucosa, characterized by multiple and bilateral nasal polyps. Different drugs have been used in its treatment. In order to study the results of different treatment modalities it is necessary to have some kind of staging. AIM: to present a new endoscopic staging method, based on nasal endoscopy and on the three-dimensional nasal polyp assessment; and compare its reproducibility with that from two other systems already established in the literature. Study design: Cohort study. Material and methods: Three experts assessed the exams of 20 patients with nasal polyposis at different times, before, at 15 and at 30 days after the start of oral prednisone, 1 mg/kg/day, during 7 days. We assessed the agreement rate among the experts, using Kappa for statistic analysis. Results: The three methods were reproducible, and the method hereby proposed had the least agreement among the examiners. Conclusions: the three-dimensional staging system proposed proved reproducible, despite showing less agreement among the examiners than the other as the other two methods.

Objective: To develop a cognitive-behavioral group therapy protocol and to verify its efficacy to reduce obsessive-compulsive symptoms. Methods: An open clinical trial with 32 obsessive-compulsive patients was performed, in which a cognitive-behavioral group therapy protocol of 12 weekly sessions of two hours, in 5 consecutive groups, was applied. The severity of symptoms was rated with the Yale-Brown Obsessive-Compulsive (Y-BOCS), Hamilton Anxiety (HAM A) and Hamilton Depression (HAM D) scales. The patients were followed up for 3 months after the end of the treatment. Results: There was a significant reduction in the scores of Y-BOCS, HAM A and HAM D scales with the treatment regardless the use of anti-obsessive medications. The rate of improved patients (decrease of > or = 35 percent in Y-BOCS) was 78.1 percent. Two patients (6.25 percent) dropped out from the study. The effect size calculated for the Y-BOCS scale was 1.75. Conclusions: This study suggests that cognitive-behavioral group therapy reduces obsessive-compulsive symptoms. ABSTRACT

The effect of G protein modulators and cyclic AMP (cAMP) on N-acetylglucosaminidase (NAGase) production was investigated during 84 h of growth of a Trichoderma harzianum strain in chitin-containing medium. Caffeine (5 mM), N6¾2'-O-dibutyryladenosine 3'5'-cyclic monophosphate sodium salt (dBcAMP) (1 mM) and 3-isobutyl-1-methylxanthine (IBMX) (2 mM) decreased extracellular NAGase activity by 80(per cent), 77(per cent) and 37(per cent), respectively. AlCl3/KF (100 µM/10 mM and 200 µM/ 20 mM) decreased the activity by 85(per cent)and 95(per cent), respectively. Cholera (10 µ/mL) and pertussis (20 µ/mL) toxins also affected NAGase activity, causing a decrease of approximately 75(per cent). Upon all treatments, protein bands of approximately 73 kDa, 68 kDa and 45 kDa had their signals diminished whilst a 50 kDa band was enhanced only by treatment with cholera and pertussis toxins. N-terminal sequencing analysis identified the 73 kDa and 68 kDa proteins as being T. harzianum NAGase in two different truncated forms whereas the 45 kDa band comprised a T. harzianum endochitinase. The 50 kDa protein showed sequence similarity to Coriolus vesicolor cellobiohydrolase. The above results suggest that a signaling pathway comprising G-proteins, adenylate cyclase and cAMP may be involved in the synthesis of T. harzianum chitinases.

OBJECTIVES: To determine prevalence of Helicobacter pylori infection in a group of symptomatic patients between 2 and 18 years old who were submitted to gastric biopsy during upper endoscopy done in the period of 1990-97. To correlate the histological findings with clinical and endoscopical data. METHODS: A cross-sectional study done after review of clinical and histopathological data. Histopathological sections were reviewed by a pathologist, who did not know the clinical information and the previous histopathological reports. RESULTS: Among 181 patients evaluated, prevalence of Helicobacter pylori infection was 24.86% (45 positive cases). In pathological analysis, gastritis was found in 38/45 of the positive Helicobacter pylori patients and in 45/136 negative Helicobacter pylori. Gastric ulceration was found in 6/45 positive Helicobacter pylori and in 3/136 negative Helicobacter pylori. CONCLUSION: This study stated a significative association between Helicobacter pylori infection and pathological abnormalities in children evaluated in the "Hospital de Clinicas de Porto Alegre", RS, Brazil.

ABSTRACT

O objetivo deste estudo foi verificar a adequação de amostras cervicovaginais colhidas para diagnóstico citológico de afecções cervicais, conforme a classificação proposta pelo Sistema Bethesda. Consiste em um corte transversal, composto por uma amostra de 4.506 relatórios de exames citopatológicos de material cervicovaginal colhidos na cidade de Barbacena e região, para realização de exames de rastreamento de câncer cervical, utilizando-se a classificação do Sistema de Bethesda. Na análise dos resultados foram utilizadas tabelas de contingência tipo RxC e o teste do qui-quadrado. O grau de significância adotado foi de 5%. Após análise estatística, a casuística avaliada foi caracterizada por 3.471 (77,03 %) amostras satisfatórias, 1.019 (22,62 %) satisfatórias mas limitadas e 16 (0,35 %) amostras consideradas insatisfatórias. Em conclusão, apesar do elevado índice de amostras satisfatórias encontrado neste estudo, esse não pode ser considerado ideal. É necessário que seja obtido número mais elevado de amostras cervicais satisfatórias para uma maior expectativa de diagnóstico precoce do câncer do colo uterino.


The objective of this study was to evaluate the adequacy of cervicovaginal samples collected for cytological diagnosis of cervical conditions, according to the Bethesda System classification. This was a transversal trial, composed by a sample of 4,506 reports of cytopathological exams of cervicovaginal material, collected in and around Barbacena, state of Minas Gerais, and screened for cervical cancer using the classification according to the Bethesda System. The results were analyzed through RxC contingency tables and chi-square test. The significance level adopted was 5%. After statistical analysis, the population evaluated was characterized by a total of 5,471 (77.03 %) satisfactory samples, 1,019 (22.62 %) satisfactory but limited, and 16 (0.35%) unsatisfactory samples. In conclusion, despite of high rate of satisfactory samples found in this study, that can not be considered ideal. It is necessary a higher value of satisfactory cervical samples to a better prospect for cervical cancer diagnosis.

ABSTRACT

Os autores compararam prospectivamente, no período de agosto de 1997 a fevereio de 1998, a eficácia entre claritromicina, azitromicina e diritromicina em esquemas tríplices para erradicação do helicobacter pylori. Após triagem pelos critérios de exclusão, foi selecionada uma amostra composta pelos primeiros 81 pacientes, não ulcerosos portadores de infecção gástrica, atendidos em postos de saúde pública. A amostra foi randomizada em três grupos: A, B e C; que receberam lansoprazol 30mg 1x/dia e amoxacilina 500mg3x/dia, durante sete dias. Os pacientes do grupo A também receberam claritromicina 500mg 2x/dia durante sete dias; do Grupo B, azitromicina 500mg1x/dia durante três dias; e do grupo C, diritromicina 500mg1x/dia durante sete dias. A propedêutica de escolha para diagnóstico e controle de tratamento foi a esofagogastroduodenoscopia eletrônica associada ao teste da Urease. O controle foi realizado dentro de 60 a 90 dias após terapêutica. A análise foi realizada com recursos de análise estatística do software Epi-info 6.04b. Três pacientes do grupo C não obedeceram à terapeutica prescrita, sendo excluidos. O índice de erradicação do grupo A foi de 85,2Por cento (23/27), no grupo C foi de13,0por cento (3/23), com p>0,001; com índices de intenção de tratamento de 85,2por cento e 11,1por cento, respectivamente(p<0,001).Dois pacientes do grupo A, três do grupo B e dois do grupo C apresentaram efeitos colaterais, sem, contudo, descontinuarem o tratamento. O esquema tríplice utilizado a claritromicina obteve níveis satisfatórios de erradicação, sendo recomendado como primeira linha para o tratamento da infecção pelo Helicobacter pylori GED 19(5):181-187,2000

ABSTRACT

A xylan-degrading enzyme (xylanase II) was purified to apparent homogeneity from solid-state cultures of Aspergillus fumigatus Fresenius. The molecular weight of xylanase II was found to be 19 and 8.5 kDa, as estimated by SDS-PAGE and gel filtration on FPLC, respectively. The purified enzyme was most active at 55 (degree)C and pH 5.5. It was specific to xylan. The apparent Km and Vmax values on soluble and indisoluble xylans from oat spelt and birchwood showed that xylanase II was most active on soluble birchwood xylan. Studies on hydrolysis products of various xylans and xylooligormers by xylanase II on HPLC showed that the enzyme released a range of products from xylobiose to xylohexaose, with a small amount of xylose from xylooligomers, and presented transferase activity.

ABSTRACT

The major cysteine proteinase from Trypanosoma cruzi, cruzipain, can be obtained essentially homogeneous, starting from crude epimastigote extracts, in one step, by affinity chromatography on Cystatin-Sepharose (specific for cysteine proteinases) or ConA-Sepharose (specific for high mannose N-linked glycoproteins). The methods offer considerable potential for enzyme purification from scarce sources, such as other parasite stages or radioactively labelled material with high specific radioactivity
